Your Advice on Telehealth Services is Needed 

Please tell us about your experience. What has it been like for you accessing services during COVID19?

• mental health 
• suicide prevention and/or
• alcohol or drug treatment

services or support in the community in the last 3 months.  You could be a person with lived experience (consumer) and/or a carer.

If you need help to complete the survey please contact your regular service provider or the My Mental Health Service Navigators on 1800 752 235 (Monday to Friday 9am to 5pm). 

Your survey is confidential.  Brisbane North PHN will use it to improve local services.  All questions are optional, and you do not have to provide any personal details that would identify you. The survey should take less than 10 minutes.
